Success rate of cataract surgery increased with advanced technology. However certain complications do occur. One of them is malposition of IOLs like decentration, papillary capture, PC IOL in AC, Subluxated IOLs, Hanging Iris claw IOL, Optics in AC, haptics in PC, Traumatic sub-conjunctival subluxation of IOL. They can cause simple blurring to sight threatening complications and requires timely intervention to prevent further complications. Approach to manage such patients is individualised according to need of each patient and cause of malposition. Surgical management included rotation, reposition of IOL and synechia release. Associated complications need additional procedures vitrectomy, anterior segment reconstruction, iris suturing and implanting IOLs like retro-fixation and aniridic IOLs. Early management not only easy to treat but also prevents sight threatening complications. This video shows surgical management of various above mentioned malposition of IOLs.
