This video presents a case of successful completion of phacoemulsification after late recognition of retrobulbar haemorrhage (RBH). Phacoemulsification was planned under local anaesthesia block as the cataract was hard and the patient was uncooperative. After making a temporal clear corneal incision, increased intraorbital thrust was felt along with gradual proptosis and conjunctival congestion and chemosis. Capsulorhexis was completed with difficulty after maintaining the anterior chamber with viscocohesive. Suspecting RBH, intravenous mannitol was started and phacoemulsification with slow motion parameters was done maintaining stable anterior chamber.
