This video presents management of a case of intraoperative hard eye due to fluid misdirection. In a 2.8 mm phacoemulsification, after nucleus emulsification, intraoperative shallowing of the anterior chamber and a marked increase in intraocular pressure (IOP) was noted without evidence of a choroidal hemorrhage. A 0.8 mm paracentesis was attempted and resultant decrease in intraocular pressure was noted. Following injection of viscocohesive, phacoemulsification was completed using slow motion phaco parameters. Early recognition with a sound diagnosis, thoughtfully assessing one’s surgical options, and proceeding with a suitable strategy for management can result in a successful surgery.
