Background: An uneventful cataract surgery is a pleasure for both patient and the surgeon. But placing IOL as secondary implantation in complication is challenging.

Method: In this video we will try to show the problems encountered by us in dealing with one of the dreaded complications i.e. capsular/zonular compromise & to place an IOL over capsular remnants. Results: The problems are related to positioning of the patient and the surgeon, deep sunken sockets, visibility status due to compromised cornea or contents of aqueous (hyphaema and/or cortical material) & post-operative inflammation, integrity of eyeball ( compromised tunnel or postoperative hypotony ), distorted iris (iris shaffereing, button hole, iridodialysis, floppy iris syndrome ), compromised zonular-capsular ( zonular dehiscence, inferior capsular loss ).

Conclusion: The assessment, anticipation and prevention of events are important to avoid difficulties later on. The surgery is joyful for both surgeon & patient.
