INTRODUCTION: Endophthalmitis is a serious complication for any intraocular surgery, endophthalmitis following Phacoemulcification and Small incision cataract surgery having different patterns.

OBJECTIVE : To highlight importance of anterior segment management in endophthalmitis vitrectomy

METHOD: Many a times, anterior segment is badly involved in endophthalmitis so much so that posterior segment management is not possible. For that anterior segment management is equally important Important steps are cataract wound management, anterior chamber management , the exudative membranes over pc iol, iris, endothelium, side port and main port has to be removed as far as possible with minimum disturbance to iris. Various techniques are displayed.

CONCLUSION: Anterior segment management is important for better visualization of posterior segment and proceed for complete surgery.
