Acute dacryoadenitis involves inflammation of the lacrimal gland and is an uncommon condition. Improper treatment can lead to lacrimal gland abscess or orbital abscess. We report a case of 7 year male child with bilateral Preseptal cellulitis not responding to oral treatment. Patient was admitted and given i.v antibiotics. CT shows asymmetric enlargement of bilateral lacrimal gland with preseptal tissue swelling. The presence of Preseptal cellulitis masked the typical eyelid findings of dacryoadenitis. This case highlights the need for imaging studies to R/o tissue involvement in case of red eyes with varying presentations
