PURPOSE : To demonstrate the retrocortical fluid which is indeed the first stage in the development of intumescent cataract.
METHODS : A 65years old female patient presented complaining of sudden loss of vision in Right eye. Her left eye was amblyopic. OCT was done before taking her for phacoemulsification cataract surgery.
RESULTS : Diffuse illumination of slit lamp showed whitish cataractous lens but slit illumination we saw clear anterior and posterior cortex behind which there was whitish fluid collection. This finding was confirmed by OCT and even on OT table after doing CCC , the surgeon tapped the nucleus and the fluid came out. The Posterior capsule was intact and foldable lens was implanted in the bag. The post op findings were also confirmed by OCT.
CONCLUSION: Retro cortex fluid accumulation is the first stage in the development of intumescent cataract. This is the first time in world literature that this has been captured with slit lamp imaging as well as OCT.
