Rathke’s cleft cyst is a fluid-filled cyst in the posterior portion of the anterior pituitary gland.
A 35-year-old presented with frontal headache of 2 months throbbing nature , diplopia on right gaze of 15 days .
visual acuity of 6/9 in both eyes , pupil bilateral reacting normally to direct and indirect light, limitation of right eye in dextroversion ,dextroelevation and dextrodepression with forced duction and forced generation tests were negative . Hess charting showed restriction of visual fields in the temporal part of right eye. Humphrey’s perimetry showed bitemporal hemianopia.
MRI- SOL in suprasellar and premescencephalic region with cisternal extension measuring 2.6 *2.9 * 3.7 cm . no calcifications noted in the lesion differentiation from craniopharyngioma.
T3 0.0.25 ng/ml, T4 3.75 μg%, TSH 10 μIU/ml ,S. prolactin 8.17 ng/ml, ACTH 17.5 ng/ml, cortisol – 5.89 µg/dl . Patient underwent transsphenoidal excision of the cyst with resolution of the symptoms.
