This is a report of a case of routine phacoemulsification in which there was a rare complication of inadvertent intrastromal air bubble formation in the corneal posterior stroma during planned intracameral air injection. This happened due to misdirection of the cannula used for intracameral air injection. Surgery was aborted and intracameral air was injected as a tamponade. The stromal air bubble gradually reduced over 3 days without any structural damage to the cornea and phacoemulsification was successfully completed.
