Occulomotor palsy during childhood is a rare condition, with an estimated incidence of 7.6 per 100,000. Sixth nerve palsy is more common amongst children than adults, and has more grave implications in terms of visual outcome. The most common causes are intra-cranial neoplasms, trauma, and less common causes being idiopathic, congenital, infections, otitis media and post vaccination.¬ It can also be seen following immunization with inactivated and live attenuated viral vaccines, and may have a recurrent course. Cases of both isolated third and sixth cranial nerve palsy following immunization with measles-mumps-rubella, diphtheria, influenza and hepatitis B have been reported. Here we report a case of an 11 month old male infant with acute benign lateral rectus palsy following MMR vaccination. There was complete resolution within 4 days with no recurrence till date.
