There are many intraoperative difficulties in a leathery hard cataract e.g. none to poor red reflex, incisional burns with a clear corneal incision, risk of PCR and zonular dehiscence, difficult to release the first nuclear piece, emulsifying hard nucleus leads to post operative stromal edema but the most important one is cracking the central leathery posterior plate and difficult to achieve a complete chop. In this video we will demonstrate the tips to crack the posterior plate in leathery hard brown black cataracts and leathery white cataracts.
