AIM: To report a rare case of unilateral congenital fibrosis of extraocular muscle, managed with series of surgeries.
METHOD: We report an unusual case of 20 years old male, presented in Occuloplasty Services of our institute with complain of deviation of left eye since birth. On detailed history & examination, exotropia >45° with associated co-contraction on convergence but no palpebral fissure height changes. OU BCVA 6/6 maintained with torticollis. Case was diagnosed as Congenital fibrosis of EOM. Stepwise three surgeries are performed, initially LR recession, followed by MR resection with temporal SR-IR fixation & finally IR recession with scar release. These efforts resulted in negative force-duction test & good cosmetic correction.
CONCLUSION: Unilateral congenital fibrosis of extraocular muscle is a rare entity, effectively managed with proper sequencing of surgeries, based on history & examination.
