We describe a rare case of povidone iodine induced reaction in a 58 years old female patient who was posted for combined surgery of right eye under local anesthesia.On the first post-operative day, the periocular area revealed complete erythema with blisters on the skin extending to the angle of mouth corresponding to the area painted by povidone iodine .Patient was treated by local dressing and short course of systemic steroids which led to its complete resolution without scarring.Prior knowledge of betadine sensitivity in patients may help prevent recurrence and and possible life threatening complications
