PURPOSE: To compare the safety, efficacy and cost effectiveness of sodium-bicarbonate mixed anesthetic mixture with hyaluronidase mixed mixture in peribulbar anesthesia.

METHOD: This prospective study was conducted on 200 patients admitted for cataract or pterygium surgeries. Patients who underwent cataract or pterygium surgeries were randomly and equally divided into two groups. A peribulbar block with local anaesthetic 2% lignocaine with 1:2,00,000 dilution of epineprine and 0.5% bupivacaine buffered with either sodium bicarbonate or hyaluronidase was given.

RESULT: Results were analyzed according to: Safety parameters:Pulse, BP, temperature, respiratory rate and oxygen saturation.
Efficacy parameters:Onset of anaesthesia, ocular akinesia, ocular anaesthesia, analgesia, lid akinesia and duration of anaesthesia.

CONCLUSION: Sodium bicarbonate can be used as an efficient and cost effective alternative for hyaluronidase in ocular regional anaesthesia.
