DSEK has been preferred over penetrating keratoplasty (PK) in the management of endothelial disorders. DSEK is most straightforward in an eye with a normal anterior segment and a stable posterior chamber IOL. Because of the well-known advantages of DSEK over PK, the indications for EK have expanded to eyes with anterior segment complexity such as those with pupillary abnormalities, peripheral anterior synechiae, glaucoma-ltering procedures, glaucoma tube shunts, prior PK, subluxated IOL, iris fixated IOLS, anterior chamber IOL and pediatric eyes. DSEK can be attempted successfully in all mentioned conditions with appropriate modifications and care to enhance the surgical outcomes. The course will be video based discussion in these
special situations.
